Register today for the final webinar of the Culture of Health series offered through Public Health Insights & Innovations. Our last presentation of the year will be on Friday, December 16, from 9:00 - 10:00 am EST, via: connect.iu.edu/phii.

Tess Weathers, faculty with IU Richard M. Fairbanks School of Public Health, will share the evidence base for addressing the social determinants of health to reduce chronic disease among vulnerable populations. Then, Brent Anderson, Medical Practice Coach with the Indiana State Department of Health, will share strategies for reducing chronic disease and health care costs by implementing Brief Action Planning and Motivational Interviewing within the context of Indiana's safety net.

Participants should expect to be able to articulate factors that support the full continuum of health and well-being, including a) enhanced individual and community well-being, b) managed chronic disease and reduced toxic stress, and (c) reduced health care costs.

We would like to thank all of our presenters who were gracious enough to share their time and expertise with their colleagues through Public Health Insights & Innovations. Without you, these brief educational sessions would not be possible.
