This form must be completed by all students who plan to graduate with an FSPH minor, certificate, or degree.
- Spring (May) Graduation – October 15 Deadline
- Summer (August) Graduation – January 15 Deadline
- Fall (December) Graduation – May 15 Deadline
How would you like your name to appear on your diploma? The IUPUI Office of the Registrar maintains the official name and address records for all students. Your name will appear on the diploma in the same format as it is stored in the IUPUI Office of the Registrar. It is your responsibility to verify that your primary name is listed in the Registrar’s Office the way you want it to appear on your diploma. If it is not, you must provide the Registrar’s Office with the corrections, including hyphens, spaces, lower/upper case letters, accents, etc. Students whose names have been legally changed or require editing should file a Request for Change of Name form with the Office of the Registrar.
Example: If the legal name is Joseph Alexander Duncan, any of the following combinations can be printed on your diploma: Joseph A. Duncan, Joseph Duncan, J. A. Duncan, or J. Alexander Duncan. The name could be shortened to Joe A. Duncan, or Joe Duncan, but nicknames such as Skip Duncan cannot be used. Names can be followed by “Jr.” or “Sr.”, but not credentials such as “MD” or “PhD” or “MS”. If you officially change your name with the IUPUI Office of the Registrar, please inform your contact person to update departmental databases.
